Fear Free Practice Pre-Visit Survey

As Fear Free certified professionals, we want to make your visit to our hospital the best it can be for you and your cat. We ask that you fill out this pre-visit questionnaire so we can take both you and your cat’s preferences for their first visit into consideration. If noted below to call, please do so for additional tips or recommendations for your cat.

During travel, does your pet do (or act) any of the following? (Select all that apply)
How would you describe your cat when at the veterinary hospital?
Check any situations listed below that your pet has shown avoidance or dislike of in the past

*NOTE For the safety and comfort of all cats and clients, we require that all cats arrive in a secure carrier or on a properly fitted harness and leash while in our lobby. If you have any difficulty catching your cat, getting them into a carrier, or preparing for travel, please give us a call at 252-772-1020. We’re happy to provide tips and recommendations to help make the process easier and less stressful.

Has your cat ever been prescribed any supplements or medications, or have any been recommended to help with a visit to the vet?
Has your cat ever needed to be sedated because of their stress level at the veterinarian office?

As part of our Fear Free approach, if your cat is too fearful, anxious, or stressed to be safely or comfortably examined, the initial visit may be converted to a “Fear Free Consult.” During this visit, we will assess your cat and may prescribe medication for you to give at home prior to a follow-up appointment. Please note that we are unable to prescribe medication before your cat has been seen and evaluated by one of our veterinarians.

Have other cats? Provide their information here and we will be happy to add them to your account!

Completing this form helps us prepare for your first visit, but does not automatically establish client status with our hospital. Client status begins once your pet has been examined by one of our veterinarians and we have agreed to assume care.
